Alert: A Message from the Practice Manager

Hi, I’m Beth the Practice Manager here at Kumar Family Practice. I have worked in General Practice for 15 years and my role is to ensure the smooth day to day running of the surgery and to make sure our patients receive the highest standard of care and service. I believe that good communication is key to providing excellent healthcare. I’m always happy to hear from patients, whether you’d like to share feedback, raise a concern or offer a suggestion about how we can improve. Care Navigation

Care navigation allows our front-line staff to provide patients with more information about local health and wellbeing services in a safe and effective way. It means that patients will find it easier to get a GP appointment when needed.

When a patient contacts the practice, the care navigator will ask for a brief outline of the problem. This is aimed to allow the care navigator to identify the patient’s need and provide information about services in the practice, other NHS providers and the wider care support sector. Where appropriate, they will direct the patient to these services. All practice staff are governed by Data Protection and Confidentiality policies.

This is designed to help give patients a choice, offering them support, and guiding them to the right information about other health care professionals who have the expertise to deal with a range of healthcare illnesses and injuries. It also supports patients to be seen by the right person at the right time.
